This program provides an allowance of $125 toward one pair of glasses every two years. The eye glasses are made by Oklahoma Optical, a tribally owned lens manufacturing business.

Specialty lenses and frames are available at a low cost to the patient.

• Prescription must be signed by a CNDH provider
