The program provides child care assistance to Native American families who are working or going to school. Notifcation of  acceptance or denial takes place within 10 days. Required application is available at the above address or can be mailed  upon request.

• CDIB
• Immunization records
• Income verification
• Class schedule and/or work schedule
• Social Security card for the child
• Utility bills
• Divorce decree (if applicable)
• Birth certificate for child

• Based on income, household size & specific needs
• Applicant must be working or attending school
• Must reside within the Chickasaw Nation boundaries
